Anonymous testing
All information regarding this test will be recorded by a random testing code number, not your name. You are given the same code number to retrieve your test results. Even if you choose to provide your name or if your testing provider knows it already, your name will not be recorded with the HIV test information nor reported to the laboratory or the Health Department.

Confidential testing
You will be asked your name which will remain in a secure location with your test counselor. Your test will be assigned a unique identifier code which consists of the third letter of your first name, the third letter of your last name and the last four digits of your social security number. Only this code will be provided to the laboratory and the department of health. Your testing provider is subject to strict rules about confidentiality.
